Occlusion-bearing Maxillary – Le Fort

  • In cases of complex trauma, it is often helpful to start with a description of involved Le Fort levels.
  • Le Fort fractures disrupt all 3 vertical maxillary buttresses, resulting in instability of the hard palate.
  • Le Fort fractures occur at 3 levels: Le Fort I (involve the piriform aperture), Le Fort II (involve the inferior orbital rim), Le Fort III (involve the lateral orbit).
  • In addition to identifying the level, also describe comminution of the hard palate and dentoalveolar involvement.
