Summary

• In cases of complex trauma, it is often helpful to start with a description of involved Le Fort levels
• Le Fort fractures disrupt all 3 vertical maxillary buttress, resulting in instability of the hard palate
• Le Fort fractures occur at 3 levels:
  • Le Fort I – involve the piriform aperture
  • Le Fort II – involve the inferior orbital rim
  • Le Fort III – involve the lateral orbit
• In addition to identifying the level, also describe:
  • Comminution of the hard palate
  • Dentoalveolar involvement
